Our first Aging in Place initiative is LTC+, a virtual care program that connects LTC nursing staff and physicians to virtual consultations with medical specialists 24/7 and advanced practice nursing. Our Aging in Place program partners with long-term care (LTC) homes and community providers to allow older adults to age at home, where they want to be. SCOPE is funded by Ontario’s Ministry of Health. The SCOPE Coordination Hub, comprised of a team at the University Health Network and Women’s College Hospital, supports the scale and spread of the SCOPE program provincially as well as conducting the evaluation of the SCOPE Program.
